Necrobiotic nodules of rheumatoid arthritis type with lipoid deposition.

چکیده
The incidence and structure of subcutaneous nodules in rheumatic fever and rheumatoid arthritis have been the subject of research for many years. In 1937 Collins published a paper on the subcutaneous nodule in rheumatoid arthritis, and in 1939 he reviewed in these Annals some of the work on the pathology of rheumatic fever and rheumatoid arthritis. He mentions the basic pattern of tissue change as coming under four heads-exudation, mesenchymal proliferation, connective tissue degeneration, and leucocytic infiltration. He remarks that this general pattern is an elementary one and cannot be regarded as specific, but that generally speaking it makes up a histological picture characteristic of rheumatism. To the connective tissue degeneration the name of " fibrinoid " degeneration has been given. This change is also part of the Arthus phenomenon, and is regarded as a basic type of tissue reaction in states of hypersensitivity. He also mentions the vascular lesions commonly found in rheumatic fever but only in the joints and subcutaneous nodules in arthritis. It seems now to be generally agreed that the typical subcutaneous nodules of rheumatoid arthritis all have a similar structure. This structure consists generally of a central area of fibrinoid degeneration and necrosis, surrounded by a cellular band, the most noticeable feature of this surround being a palisade arrangement of fibroblasts. The case to be described in this article falls only partly into this category, and it should be mentioned that Parkes Weber published a somewhat similar case, though differing in several features, in the Annals of 1944.

Subcutaneous Nodules of Rheumatoid Arthritis with Lipoid Deposition.
Weber and Freudenthal (1936-7), Weber (1943, 1944, 1947, 1948), and Fletcher (1946, 1947) have noted the rare occurrence of subcutaneous nodules in rheumatoid arthritis which consisted almost entirely of lipoid, and have designated such cases by various names to differentiate them from the " common " type of rheumatoid arthritis subcutaneous nodule. During the clinical examination of seventy ca...
متن کاملCardiac necrobiotic (rheumatoid?) granulomas without arthritis. Report of two cases.
Cardiac necrobiotic lesions resembling rheumatoid granulomas were discovered in 2 patients without evidence of arthritis. Evidence from the literature indicates that pulmonary and subcutaneous nodules may occur before arthritis or before the appearance of the rheumatoid factor, and that cardiac lesions may occur early in the course of rheumatoid disease. Even in the absence of obvious arthritis...
متن کاملCutaneous manifestations in patients with rheumatoid arthritis
Rheumatoid arthritis (RA) is a chronic inflammatory joint disease with different extra-articular manifestations. Among those extra-articular manifestations, there are significant cutaneous manifestations which may result in first admission to a dermatologist. In this article some specific and non-specific skin lesions of RA such as rheumatoid nodules, exacerbated nodules, rheumatoid vascu...
متن کاملMassive cavitary pulmonary rheumatoid nodules in a patient with HIV.
The case of a 52-yr-old female with rheumatoid arthritis and HIV who developed massive, progressive, cavitary pulmonary nodules is described. Multiple diagnostic bronchoscopies and lung biopsies failed to demonstrate the presence of any microorganisms. Pathological analysis showed palisading histiocytes with necrobiosis consistent with rheumatoid nodules. The effect of co-existing HIV infection...
متن کاملPulmonary necrobiotic nodules as a presenting feature of rheumatoid arthritis.
In rheumatoid disease only 4 cases have been reported in which lung nodules preceded arthritis. Two further cases are presented in which intrapulmonary nodules were noted at the onset of the disease, thus provoking a diagnostic dilemma. In the first patient rheumatoid factor tests were initially negative, and diagnostic thoracotomy was performed. In the second patient serological tests were pos...
